Immunisation Records

Comprehensive immunisation records are kept by the City of Ararat. Records are available held for 20years. Copies of these records are available by contacting immunisation co-ordinator on 03 5352 1813.

Also immunisation records can be obtained from the Australian Childhood Immunisation Register (ACIR) which is a national database that keeps a record of your children’s' immunisations up to the age of seven years. Immunisation providers (Council or your doctor) report your child’s immunisations to the ACIR. A history statement of all of your child’s immunisations is automatically posted to you at one, two and five years of age.

The Australian Childhood Immunisation Register now supplies a history statement that acts as a school entry immunisation certificate.

The statement is automatically posted to you once your child has completed the four year old immunisations and has completed all required childhood immunisations. For more information, please contact ACIR on 1800 653 809.

If for some reason your child’s immunisation records are incomplete, missing, or your child has never been vaccinated, contact Council's Immunisation Coordinator (on 03 5352 1813) who will be able to assist you in obtaining a school entry immunisation certificate.
